How does the auto-writeoff (apost) flag on the patient demographic screen work?

September 17th, 2019

What is the Auto-writeoff flag?

This flag by default is always set to N and only affects patient charts where Medicare is their primary insurance.  By default, this flag will always be set to N (Normal Patient Bill.)

Selecting Y (MCR W/Off Pat Bal) this will write-off PATIENT balance when Medicare primary payments are applied from AutoPost.  If the patient has a secondary insurance on the chart/claim for a patient this flag will be ignored. This also ignores the setting when a deductible is applied by Medicare and will be balance billed to the patient.

  • If there is a secondary insurance (except Medicare DMERC)  on the chart, the balance will be moved to the SECONDARY.
  • If the secondary insurance on the chart is Medicare DMERC, the balance will be written -off.
  • If the secondary insurance is DMERC and there is a third  insurance entered on the chart, the balance will be written-off.
  • If the Medicare Primary payment was applied to the patient’s deductible, the balance will not be written-off and will be billed to the patient or secondary insurance.

*There is another exception for Florida providers.  If Medicare is the primary and Medicaid is the secondary, the Auto-writeoff flag is not required to be set to Y.  Autopost will automatically write-off the balance if the primary is Medicare and secondary is Medicaid and the balance was not applied to the deductible.  This can be overriden by checking feature flag 122, FL Medicaid - No Autopost w/o.

Additional Write-off options

These options were added to the Autopost Program with the release of version 5.7.7.758 on August 15th, 2018.

Apost Writeoff: Facility Only Claims : When Facility Only Claim is checked, it will write off patient balances that are associated with any place of service that is NOT office (11) or home (12).

Apost Writeoff: Include Deductible:  When Include Deductible is checked, it will write off patient balances including any deductible amounts  The flag will ignore deductibles as an outstanding patient balance.

Each of these settings may be selected or managed separately.  These settings only apply to Medicare Primary patients set to Auto Writeoff = Y.
