EPCS Requirement for NY Providers

Please complete the Sammy EPCS Form located here.

If you are a provider in New York follow the instructions below. 

  1. Once you have completed your 2 factor authentication, you will need to register your software with the NYS Department of Health.
    1. When you complete the DOH application online, you will need the following information.
      • Name of certified e-Prescribing software application: NewCrop
      • Software Version Certified: SammyEHR/SammyUSA/SimpleSam (whichever applies to you)
      • Name of Software Application Provider: ICS Software, Ltd.
