If this page is printing blank, then you probably do not have a form created in the Forms Library for the doctor in the Physician of record on the chart.

Go to main Sammy menu and select, Libraries, Forms Library.

Select the doctor on the right side and then Double click on "Insurance Authorization Form' on the left side.

If the window is blank, the form will print a blank page. If you have a form created for a different doctor in your practice, you can copy text from another doctor to paste to here. Or simply add the text you would like to be printed on this form.
